United Healthcare / Golden Rule Contracting

United Healthcare / Golden Rule individual major medical products are currently available in all states except the following:

HI / ID / ME / MA / MN / MT / NH / NJ / NY / ND / OR / RI / UT / VT / WA

If you reside in one of the 15 states where Golden Rule is not available contact us to discuss how you can easily and electronically obtain non-resident licensing in one or more of the 35 approved states in a matter of minutes.

If you had previously contracted with Golden Rule directly or through another organization you will not need to pay appointment fees.  All you need do is complete 2 Golden Rule contract pages to transfer your Golden Rule contract to us.  Click here to download the 2 pages.  Just complete those portions with a green check mark beside it.

Golden Rule appointment fees are collected via a credit card form included in the 9 page contracting document above.

Here is your Assurant and United Healthcare / Golden Rule checklist.  After you have:

#1: Paid your appointment fees for Assurant plus the $429 initial lead, set up and training fee using the Paypal portal on the previous Assurant contracting page. 

#2. Filled out the 4 Assurant signature pages you downloaded on the previous page.

#3:  Filled out the 9 pages of United Healthcare / Golden Rule contracting (or 2 pages if you have previously been appointed with UHC/GR).

Return the 4 Assurant signature pages and 9 Golden Rule contracting pages for a total of 13 contracting pages (or a total of 7 pages if you have previously been appointed with UHC/GR) along with a copy of your license or licenses if being appointed in multiple states, a copy of your void check if you wish to be paid electronically on a DAILY BASIS on Assurant and Golden Rule business.  You may send your Assurant and Golden Rule contracts to us by either one of two methods.

#1:  If you are capable of scanning your Assurant and Golden Rule contracts into pdf format (adobe acrobat reader) please email your contracts with pdf attachment to: wroche@policyshopper.com

#2: If you are not capable of sending via option #1 please fax your contracting to:

Only return back to us the 4 Assurant signature pages plus the 9 Golden Rule contracting pages (total of 13) PLUS a  copy of your state insurance license or licenses!
